肝右葉を占居する巨大囊胞性腫瘍（径20 cm）に対して肝右葉切除を施行した後に残肝再発巣を切除した肝血管筋脂肪腫の1例を報告した．症例は腹痛で発症した50歳の女性である．初発病変は巨大な多房性腫瘍で，囊胞液細胞診は陰性，非血性であったが壁在結節や壁肥厚を認めたため，悪性を念頭に肝右葉切除を行った．組織学的には腫瘍の大部分が平滑筋様細胞であり，血管成分も認められたが脂肪成分は認めなかった．さらに，HMB-45とmelan A陽性なことから肝血管筋脂肪腫と診断した．術後8か月後に出現した径1.5 cmの囊胞は1年後に4 cmに増大したため再肝切除を施行した．組織は前回と同様であった．本例は腫瘍の大部分が多房性囊胞の肝血管筋脂肪種であった．巨大であり急速増大や残肝再発などからmalignant behaviorの存在が示唆された．再発切除後48か月無再発であるが，今後も慎重な経過観察を要する．
