Changes in Pediatric Hospital Staff’s Sense of Difficulty Toward Palliative Care:A Single-pediatric Hospital Survey Report

本研究は，小児専門病院の緩和ケアチームが院内コンサルテーションを開始する前後で院内スタッフの緩和ケアに対する困難感の変化を明らかにすることを目的に行った．5領域21項目で構成される困難感に関する自記式質問紙を用い，2015年に開始前，3年後に開始後調査を実施した．開始前は222名（回収率70.9%），開始後は384名（回収率87.3%）から回答を得た．回答者の7割以上が看護師・助産師であった．全職種では，“苦痛症状の緩和”，“看取りの際の家族ケア”，“自分自身や周囲のスタッフが感じる不全感や喪失感に対する支援”の3項目で困難感の有意な減少が認められた．介入した部署の看護師・助産師の困難感は6項目で有意な減少を認めた．緩和ケアチームが介入した16件中11件が疼痛コントロール難渋例の2名の患者への複数回の介入依頼であり，コンサルテーション活動が看護師・助産師の困難感の減少に寄与したと推察する．

Medullary and upper cervical cord infarction caused by occlusion of the unilateral vertebral artery

症例は80歳男性．入浴後に意識障害，眼振，構音障害，右上下肢麻痺を発症し救急搬送となった．頭部MRIでは左頭頂葉皮質下の微小急性期梗塞を認め，magnetic resonance angiography（MRA）では左内頸動脈狭窄を認めたため，アテローム血栓性脳梗塞と診断し入院となった．入院翌日，吸痰中に突然意識レベルがJCSIII-300に低下し呼吸停止となった．気管挿管，人工呼吸器管理を開始し意識レベルの改善を認めたが，四肢麻痺が持続した．入院4日目に施行した頭頸部MRIでは右延髄～上位頸髄にかけての長軸進展病変を認め，computed tomography angiography（CTA）で右椎骨動脈閉塞の所見があり，梗塞と診断した．片側椎骨動脈閉塞で上位頸髄梗塞を来す可能性があり，注意が必要である．
