A case of schwannomatosis with right vagal nerve schwannoma

52歳女性．左鎖骨上窩の腫瘤を主訴に当院外科を受診した．胸部CTで両側頚部，左鎖骨上窩，そして右後縦隔に約8cmの腫瘍を認めた．当初悪性腫瘍のリンパ節転移なども疑い，全身検索と腫瘍の質的診断を行った．FDG-PETでは両側頚部，左鎖骨上窩，右後縦隔，左後腹膜，左大腿部に集積を認め，各部位のCTおよびMRIによる質的診断では，全病変で神経鞘腫が疑われたため，右後縦隔腫瘍に対し，胸腔鏡補助下腫瘍摘出術を施行した．術中所見では右迷走神経の走行に一致して腫瘍を認めた．摘出標本の最終病理診断は迷走神経鞘腫であった．Café-au-lait-spotsはなく遺伝歴もないため，本症例の最終診断は，多発性神経鞘腫症と診断した．多発性神経鞘腫症で迷走神経鞘腫を合併することは稀である．FDG-PETは，全身の腫瘍部位を同定する検査方法として有効な手段であると考えられた．
