A Case of Localized AL Amyloidosis in a Patient with Sjögren's Syndrome

70 歳台，女性。既往症に関節リウマチとシェーグレン症候群があった。初診 5 カ月前に右下腿に硬結が出現した。初診時，両下腿の 3 カ所に結節と腫瘤があり，80×64 mm の一部潰瘍化した紅褐色腫瘤が最大の病変であった。病理組織学的に，真皮に好酸性の無構造物質の沈着があり，Congo red 染色が陽性であった。免疫組織化学染色でアミロイドは免疫グロブリンλ鎖と同定した。全身精査で異常はなく，シェーグレン症候群に伴う限局性皮膚結節性アミロイドーシスと診断した。
