要旨：アルコール依存症で精神科入院歴のある45 歳男性．右片麻痺および痙攣で入院．両側頭頂葉皮質下に出血を認め，造影CTで上矢状静脈洞に血栓による閉塞を確認した．原因となり得る先天的酵素欠損や膠原病，血管炎，感染症，悪性腫瘍等を認めず，前医で処方されていたクエチアピンの関与を疑い，これを中止した．抗凝固療法を継続し，血栓の退縮を認め，第62病日に転院．抗精神病薬の重大な副作用に静脈血栓塞栓症があり，近年は非定型抗精神病薬によるリスクが注目されている．特にクエチアピンのリスクの高さを指摘する報告があり，使用に当たっては常に静脈血栓塞栓症のリスクを念頭に置く必要がある．

A Case of Microscopic Polyangiitis with Hypertrophic Pachymeningitis and Diabetes Insipidus

症例は57歳，女性．3年前より紫斑，血尿で発症した顕微鏡的多発血管炎（microscopic polyangiitis：MPA）で加療されていたが，尿崩症，頭痛，脳神経麻痺が出現，CRP，MPO-ANCAが再上昇した．胸部CTで肺多発結節影，脳MRIで硬膜肥厚，下垂体腫大を認め，ステロイド増量，免疫抑制剤投与で改善した．本例は当初顕微鏡的多発血管炎に典型的であったが，多発血管炎性肉芽腫症に病態が移行した．また，MPO-ANCA陽性血管炎に尿崩症を来たすことは極めて稀であり，貴重な症例と考えられた．
