A case of β-catenin-activated hepatocellular adenoma with TERT promoter mutation in a young woman not taking oral contraceptives

経口避妊薬の服用歴のない37才女性.健診で肝腫瘤を指摘され当施設紹介.腹部超音波検査で肝に径8 cmの結節内結節構造を呈する高エコー腫瘤を認めた.MRI-T1Wで低信号,T2Wで等信号.血管造影下CTの動脈早期相で多血性,後期相で低吸収域となり,周囲にコロナ様の濃染を認めた.肝針生検標本では異型に乏しい肝細胞から成り,免疫組織化学的にβ-catenin核内集積を認め,β-catenin活性化型肝細胞腺腫(b-HCA)と診断された.肝右葉切除術施行.切除標本には結節内結節構造を呈する径9.8 cm大の腫瘤を認め,遺伝子解析の結果,内側の結節のみにTERT promoter領域の遺伝子変異を認めた.本症例は経口避妊薬非服用女性に発生したb-HCAで,結節内結節構造の内側結節のみにTERT promoter領域の遺伝子変異が認められ,b-HCAの悪性転化を考える上で興味深い症例と思われたので報告した.
