症例は65歳の男性で，背部痛精査の造影CTで遠位胆管癌を認め，胆汁細胞診にてClass Vであった．幼小児期に複数回の開腹歴と大量腸切除の既往から食事摂取は可能であったが，短腸が推測され，また高尿酸血症・高血圧・高脂血症・慢性腎障害を認めた．短腸症例に膵切除を施行する場合には栄養障害の可能性が考えられることから，Synapse Vincentを用いて残存小腸の長さを測定し手術適応を決定した．手術は亜全胃温存膵頭十二指腸切除を行った．術後は消化吸収の点から中心静脈栄養管理を主体に開始し，成分栄養剤にて腸管機能の改善を確認後に膵酵素剤を併用し食事を開始した．短腸の症例に対する膵頭十二指腸切除は報告がないが，その適応は残存膵機能の面と残存小腸機能の面から慎重に検討することで施行可能であると考えられた．

A case of β-catenin-activated hepatocellular adenoma with TERT promoter mutation in a young woman not taking oral contraceptives

経口避妊薬の服用歴のない37才女性．健診で肝腫瘤を指摘され当施設紹介．腹部超音波検査で肝に径8 cmの結節内結節構造を呈する高エコー腫瘤を認めた．MRI-T1Wで低信号，T2Wで等信号．血管造影下CTの動脈早期相で多血性，後期相で低吸収域となり，周囲にコロナ様の濃染を認めた．肝針生検標本では異型に乏しい肝細胞から成り，免疫組織化学的にβ-catenin核内集積を認め，β-catenin活性化型肝細胞腺腫（b-HCA）と診断された．肝右葉切除術施行．切除標本には結節内結節構造を呈する径9.8 cm大の腫瘤を認め，遺伝子解析の結果，内側の結節のみにTERT promoter領域の遺伝子変異を認めた．本症例は経口避妊薬非服用女性に発生したb-HCAで，結節内結節構造の内側結節のみにTERT promoter領域の遺伝子変異が認められ，b-HCAの悪性転化を考える上で興味深い症例と思われたので報告した．
