Successful Treatment of Chronic Active Epstein-Barr Virus Infection Complicated by Cardiomyopathy in an Adult Patient

Chronic active Epstein-Barr virus infection (CAEBV) is a rare and potentially fatal condition involving EBV-infected T or natural killer cells. We herein report a 46-year-old man with CAEBV complicated by cardiomyopathy. Imaging and a biopsy revealed EBV-positive CD3+ T cell infiltration in the myocardium. The patient was treated with high-dose cytarabine-based chemotherapy followed by cord blood transplantation using reduced-intensity conditioning, resulting in complete donor chimerism. Although transient cardiac dysfunction and post-transplant EBV reactivation occurred, the patient remained in remission for five years. This case underscores the importance of a myocardial biopsy and long-term monitoring of CAEBV with cardiac involvement.
