Plasma-lipoprotein Patterns in Coronary and Cerebral Vascular Diseases, Gallstones and Obesity

It has been established that there is a close relationship between hypercholesterolemia and coronary heart disease. But cerebral vascular disorders (arteriosclerosis and thrombosis) are rather common in area where people live in poor nutritional conditions. The serum-cholesterol level in cerebral thrombosis is not always increased above the normal level. In the present investigation the distribution of cholesterol and phospholipids between α and β-lipoproteins was determined in patients with coronary heart disease, cerebral vascular disorders, gallstones and obesity, and the abnormality in composition of plasmalipoproteins was examined.Lipoproteins were separated as heparin-metal complex by the method of Burstein and cholesterol and phospholipids were determined for α and β-fractions. Lipoprotein analysis was also carried out on paperelectrophoresis with cellulose acetate film. The results obtained with the quantitative determination agrees satisfactory with the results on paper electrophoresis.Total serum-cholesterol, triglycerides and phospholipids were all significantly increased in coronary heart disease. However there was no significant changes in serum-cholesterol in the other groups. Lipoprotein analysis showed that there was a significant increase in β-lipoprotein cholesterol and phospholipids in coronary disease as well as in cerebral vascular disorders, while there was no change in patients with gallstones and obesity. α-Liporotein was markedly decreased in patients with coronary disease and cerebral vascular disorders, so that the distribution ratio of cholesterol between β and α lipoproteinb (β/α ratio) was markedly increased even when the total serum-cholesterol was in normal range. The decrease in α-lipoprotein and the increase in β/α ration were also observed in patients with gallstones and obesity. Atherosclerosis and gallstones are common complications of obese patients. It is supposed that these diseases are based on the same metabolic abnormality in plasma lipoproteins. The decrease in α-lipoprotein has recently been reported by Yamanaka et al. and by Miller and Miller.Electrophoresis of lipoproteins also revealed-another abnormality in patients with hypercholesterolemia. Ordinarily, preβ-band moves a half way between α and β-bands on cellulose acetate film. But in hypercholesterolemia preβ-band appeared close to β-band with a slow migration. This band was called as preβ-1 (Dahlen) and is probably the same as the one called as midband (Wada and Mise) or extra β-band (Naito) on PAG.Cholesterol β/α ratio must be used as an important factor for determining a covert hypercholesterolemia.
