A Case of Recurrent Adenocarcinoma Component Following Treatment of Combined Small-cell Lung Cancer Successfully Treated with Cytotoxic Chemotherapy Combined with Immune Checkpoint Inhibitors

背景．混合型小細胞肺癌（combined small-cell lung cancer：C-SCLC）は，小細胞肺癌（SCLC）に非小細胞肺癌（NSCLC）成分を含む稀な腫瘍で，治療法や予後は確立されていない．症例．71歳男性，検診で胸部異常影を指摘され受診．画像検査と超音波気管支鏡ガイド下針生検（endobronchial ultrasound-guided transbronchial needle aspiration：EBUS-TBNA）で右肺上葉の限局型C-SCLC（cT1bN2M0，cStage IIIA）と診断．限局型SCLCに準じてシスプラチン＋エトポシドと加速過分割照射による化学放射線療法（CCRT）を実施し，部分奏効を得た．治療終了1年後，CEA値の上昇と右気管傍リンパ節の腫大を認め，NSCLC成分の再発を疑った．再度のEBUS-TBNAでNSCLC再発と診断，カルボプラチン＋nab-パクリタキセル＋デュルバルマブ＋トレメリムマブを4コース施行した．CEA値の低下と病変縮小を認め，現在も免疫関連有害事象なく治療を継続中である．結論．本症例は，C-SCLCに対するCCRT後のNSCLC成分再発に対して複合免疫療法が有効であった．C-SCLC症例では適宜生検と遺伝子解析を行い，その時々で適切なレジメン選択をしていくことが重要である．
