Efficacy of the Altemeier Procedure for Rectal Prolapse

目的:直腸脱に対するAltemeier法の有効性と安全性について検討した.方法:2009年12月から2024年12月までにAltemeier法を施行した120例を後方視的に解析した.また同時期に施行した直腸固定術(Rectopexy)と比較検討した.結果:平均年齢は81歳,女性は117例であった.Clavien-Dindo分類II以上の合併症はAltemeier法で4.2%,Rectopexyで6.8%に発生した.再発率はAltemeier法で38%,Rectopexyで6.8%であり,再度Altemeier法を受けた5例全員が再発した.再発のリスク因子は女性,手術時間,直腸脱の手術歴であった.結論:Altemeier法は低侵襲かつ安全な術式であるが,Rectopexyと比較して再発率が高い点が問題である.再発例には経腹手術の適応を考慮すべきである.
