A Case of Obstructive Jaundice Due to Hepatic and Hepatoportal Lymph Node Metastases of Breast Cancer

黄疸を呈する乳癌肝転移は予後不良で，積極的な治療が行われることは少ない．今回われわれは，経皮経肝胆道ドレナージ（PTCD）によって減黄が得られ，通院による経口抗癌剤投与で高いQOLを保ちつつ生存期間の延長が得られた乳癌肝転移・肝門部リンパ節転移の１例を経験したので報告する． 症例:52歳，女性．左乳癌T1cN3cM1（頸部リンパ節），ER陽性，PgR陰性，HER2 1+の診断にて術前化学療法（EC followed by docetaxel療法）を施行しPRを得たのち，乳房部分切除術および腋窩郭清を施行した．術後はanastrozoleを投与していたが11カ月後に多発骨転移，20カ月後に多発肝転移・肝門部リンパ節転移を認めた．閉塞性黄疸をきたしたが，PTCDで速やかに減黄し肝機能は回復した．その後S-1を投与したところ奏効し，９カ月の無増悪期間を得た．乳癌肝転移に伴う黄疸でも，その原因が肝門部リンパ節転移による閉塞性黄疸であれば，積極的にPTCDで減黄し化学療法を行うことで患者の予後を向上させ得る可能性が示唆された．
