Clinical Characteristics of Type D Ventilation Tubes in the Management of Otitis Media with Effusion in Children

難治化リスクを伴わない小児滲出性中耳炎の初回鼓膜チューブ挿入術において, ガイドラインでは短期留置型チューブを第一選択として推奨している. 今回, 短期留置型鼓膜チューブ (D型) を用いて鼓膜チューブ挿入術を行った15歳未満の小児69例122耳を対象とし, 再発率, 穿孔残存率, 感染率について, 留置期間や中耳貯留液の性状, 血清学的指標との関連について検討した. 平均留置期間は15.7カ月で, 再発率は13.1％, 穿孔率は6.0％, 感染率は7.4％であった. 再発の有無での２群間比較を行うと, 再発なし群で有意に留置期間が長かった. また, 血清学的指標と中耳貯留液の性状, 再発率の間に明らかな傾向は認めなかった.

Postoperative complications of parotidectomy for benign parotid tumors

耳下腺手術の合併症には，顔面神経麻痺・Frey症候群・唾液瘻等があり，特に顔面神経麻痺はQOLに直結する注意すべき合併症である。当科で2012年4月から2020年6月に手術を施行した耳下腺腫瘍350例のうち，良性腫瘍の新鮮例273例を対象として術後合併症につき検討した。術後合併症は顔面神経麻痺が18例，唾液瘻が13例，Frey症候群が3例であった。前期群と後期群に分けて比較したところ，後期群で合併症の頻度が低かった。顔面神経麻痺を認めた症例の腫瘍の局在は，浅葉が8例，下極が4例・深葉が6例であった。単変量解析では，深葉の場合に麻痺が有意に多い結果であった（p＝0.009）。深葉の症例の手術の際は特に顔面神経麻痺に注意する必要がある。

Surgical Resection of Carotid Body Tumors: a 17-year Experience

慶應義塾大学病院で手術加療を行った頸動脈小体腫瘍23腫瘍の検討を行った. 死亡や脳梗塞といった重大な合併症はなく, 術後の神経障害も諸家らの報告と比較し少ない結果であった. 手術適応を吟味した上で脳外科・血管外科とのチーム医療を行うことで安全な手術を提供できた結果となった. また腫瘍が内頸動脈を取り囲む角度を術前に計測することで, 頸動脈再建の要否をある程度推測することが可能であった.
