直腸肛門部悪性黒色腫 (ARMM) の発生頻度は稀であり, 極めて予後不良である。症例は53歳, 男性。下血を主訴に当科を受診。直腸診にて直腸後壁に弾性軟の腫瘤を触知し, 注腸造影検査および大腸内視鏡検査にて直腸 (Rb) に易出血性で横径約2.5cmの隆起性病変を認めた。2度にわたる生検時の病理診断がGroupIIIであったため, 確定診断目的にpolypectomyを施行したところ悪性黒色腫の診断を得た。深達度はmp, 断端陽性であった。Microwave coaglation therapy (MCT) とcisplatin, vindesine, dacarbazineからなるCVD療法を開始した。20カ月を経過した時点で腫瘍の増大を認め, 疼痛が増強してきたため, 経肛門的腫瘍切除, 人工肛門造設術を施行。23カ月目に肺, 肝転移を認め, 27カ月時に原病死となった。本邦ではARMMの大多数に対して腹会陰式直腸切断術が選択される。本症例は原発巣の局所コントロールと化学療法により, 従来の報告例と比較して長期生存を示した1例である。

症例は32歳, 女性。12歳時に甲状腺乳頭癌の診断にて甲状腺右葉切除+右頸部リンパ節郭清術を受けている (T2, N1, M0=StageI) 。術後20年目の定期検査時, 超音波検査で甲状腺左葉に直径9.8×9.2mmの粗大石灰化を含むlow echoic massを認め, 直径10mm程度の左頸部リンパ節の腫大を数個認めた。CTでも残存甲状腺内に同様の所見を認め, 気管前面に腫大したリンパ節を認めた。リンパ節の穿刺吸引細胞診を施行したところ, Class IVであった。以上より, 残存甲状腺の腫瘍とリンパ節転移と診断し, 残存甲状腺全摘術+左頸部リンパ節郭清術を施行した (Tx, N1b, M0=stage I) 。リンパ節の病理組織診断は乳頭癌であったが, 残存甲状腺腫瘍はcalcifying noduleの診断であった。術後経過は良好であり, 第6病日に退院した。頻度が稀で, 比較的予後の良いことで知られる小児甲状腺癌であるが, 本症例のごとく20年後にリンパ節再発をきたすことがあるので, 長期にわたる経過観察が重要であると考えられた。
