A Case of Intestinal Endometriosis Coexisting with the Canal of Nuck

症例は53歳の女性．Nuck管水腫の診断で切除術を施行され，病理組織学的に子宮内膜症を伴ったNuck管水腫と診断された．初回手術から26カ月後，他疾患のフォロー目的に施行した腹部造影CTで偶発的に回盲部の壁肥厚を指摘された．診断，治療目的に腹腔鏡下回盲部切除術が施行され，病理組織学的に腸管子宮内膜症と診断された．初回手術前の腹部単純CTを確認すると，回盲部に壁肥厚を認め，Nuck管水腫と腸管子宮内膜症が併存していたことが明らかとなった．Nuck管水腫は，Nuck管内やその近傍の鼠径部，あるいは女性生殖器に子宮内膜症を併存するとされるが，その他臓器への併存は報告がない．自験例から，Nuck管水腫に併存する子宮内膜症は腹腔内の他臓器にも発症する可能性が示唆され，Nuck管水腫診療において，術前，術中の腹腔内検索を考慮する必要性があると考えられた．

Two Cases of ALK Fusion Gene-positive Lung Adenocarcinoma Treated with Brigatinib After Alectinib-induced Interstitial Lung Disease

背景．アレクチニブはALK融合遺伝子陽性肺腺癌に対して有効性や忍容性が高い薬剤であるが，有害事象の間質性肺疾患をきたした場合，治療を継続することができない．他に数種類の使用可能なALK阻害薬があるが，ブリグチニブは近年使用可能になったALK阻害薬である．アレクチニブによる間質性肺疾患後にブリグチニブで治療し得たという報告はなく，今回我々は2例を経験したため報告する．症例．2症例はいずれもALK融合遺伝子陽性肺腺癌であり，アレクチニブ開始後にそれぞれgrade 2とgrade 3の間質性肺疾患を認めた．ステロイドの全身投与で治療し間質性肺疾患の改善後にブリグチニブを開始したところ，1例では間質性肺疾患の再発なく抗腫瘍効果が得られた．高齢の1例では間質性肺疾患が再発したがブリグチニブの休薬のみで間質性肺疾患は軽快し，ブリグチニブを減量したところ間質性肺疾患の再発なく抗腫瘍効果が得られた．結論．アレクチニブによる間質性肺疾患発現例において，ブリグチニブへの変更は治療選択肢となり得る．しかしALK阻害薬に対する間質性肺疾患の既往がある患者の場合は注意深くフォローアップする必要がある．
