加熱式タバコ誤飲による消化管異物の一例

近年日本国内に広く普及した加熱式タバコの誤飲が相次いで報告されている．金属片を内蔵した加熱式タバコを誤飲した0 歳11 か月児の症例を経験したため報告する．患児が加熱式タバコを口に咥えているところを発見され救急搬送で来院した．胸腹部X 線検査を施行し胃内に12 mm×4 mm 大のX 線非透過性の異物を認めた．頻回に嘔吐が続き，上部消化管内視鏡での緊急摘出も検討されたため，高次医療施設へ転院搬送となった．搬送中に嘔吐は改善したため，自然排出が期待され，経過観察入院の方針となった．翌日に異物は腸へ進行しており，同日夜に便と共に異物が排泄された．近年市販されている加熱式タバコはニコチン中毒に加え，金属片による消化管異物に陥る危険性がある．金属片は薄く角が鋭利であり，組織を損傷する可能性があるため，緊急内視鏡での摘出を要する場合もありうる．今後は危険性や誤飲防止について保護者へ広く啓蒙することが必要と考えられる．
