高齢者介護施設で適切な薬剤耐性菌対策を行うために，特別養護老人ホーム（施設A～C）を対象に糞便検体から基質特異性拡張型β-ラクタマーゼ（Extended-spectrum β-lactamase：ESBL）産生菌保菌の実態調査および保菌と利用者特性・施設特性との関連因子の解析を行った．その結果，利用者特性では抗菌薬投与群のESBL産生菌検出率が有意に高く（検出率の比12.71［95%CI：1.33-121.53］），ESBL産生菌保菌と抗菌薬投与歴の関連が示唆された．施設特性では，介護度が最も低い利用者が入所しているユニット型（個室型）施設（施設B）の検出率（36.7%）が最も高く，介護度の高い利用者が入所している従来型（多床室型）施設（施設C）の検出率（5.9%）が最も低かった．両施設の違いは，施設Bでは日常的に抗菌薬を処方されている利用者がいるのに対し，施設Cでは抗菌薬が全く処方されていないことであった．高齢者介護施設は薬剤耐性菌拡散に対して大きな役割を果たしていると考えられてきたが，施設を担当する配置医師の抗菌薬処方状況によって薬剤耐性菌の保菌リスクは大きく異なると推察された．

新型コロナウイルス感染症（COVID-19）流行期に首都圏（東京・神奈川・千葉・埼玉）の精神科病院および介護福祉施設（介護老人保健施設［老健］と特別養護老人ホーム［特養］）を対象とし，感染対策組織の設置状況や実施した措置，各種介助時や医療処置時に装着した個人防護具（personal protective equipment：PPE）のアンケート調査を行った．その結果，感染対策委員会は設置している（設置率86.7%）が，感染対策の専門家は不在であることがわかった（配置率9.1%）．近隣の医療機関との情報共有についての実施率は精神科病院が50.0%（24/48），老健が59.5%（22/37），特養が42.4%（14/33）であり，約半数の病院・施設で行われていなかった．
