Factors influencing the improvement of aphasia in patients with left putaminal hemorrhage in the subacute phase

【背景および目的】回復期における左被殻出血によって生じた失語症の書字・発話・言語理解の改善に影響を与える要因を検討した．【方法】対象者70名の診療録から，年齢，性別，入院期間，血腫型，血腫量，NIHSS, レーヴン色彩マトリシス検査（以下，RCPM），失語タイプ，失語症重症度を後方視的に調査した．標準失語症検査総合評価尺度の下位項目である書字・発話・言語理解の得点の改善群と非改善群において差のある要因を検討した．【結果】両群において，発話は年齢，NIHSS, RCPMに有意差を認め，書字では発話と同様の要因に加えて血腫量に有意差を認めた．言語理解は2群間で差のあった要因は認められなかった．【結論】書字や発話といった表出面の改善には，脳卒中の重症度および知的機能の保持が寄与し，言語理解は要因にかかわらず，改善しやすい言語モダリティであることが示唆された．

The characteristics of aphasia by hematoma type in patients with left putaminal hemorrhage admitted to comprehensive inpatient rehabilitation wards

【背景および目的】回復期リハビリテーション病棟に入院した左被殻出血の発症時CT画像所見から，血腫型による失語症重症度の特徴を見出すことを目的とした．【方法】対象者92名の診療録から，年齢，性別，発症から入院までの期間，発症時CT画像所見における血腫型と血腫量，神経徴候，言語機能を後方視的に調査し，血腫型（I～V型に分類）とSLTA総合評価尺度および下位項目である書字・発話・言語理解との関連性を検討した．【結果】失語症の重症度について，I型は軽度，IV・V型は重度となる傾向があったが，II・III型に明らかな傾向はなかった．下位項目をみると，I型では書字のみ低下，IV型は発話と書字での著明な低下，V型は全ての項目で著明な低下を示した．【結論】血腫型における失語症重症度や下位言語モダリティには血腫量の影響が大きく，血腫の進展による白質損傷が失語症重症度に関与する可能性が示唆された．
