A Case of Idiopathic Gastric Volvulus Resulting in Gastric Perforation

症例は72歳，男性．5年前に胃軸捻の入院歴があり，保存的加療で軽快していた．今回，嘔吐と上腹部痛で前医に救急搬送され，胃軸捻と診断された．胃管留置による減圧で症状改善なく，2時間後の腹部骨盤造影CTの再検では上腹部にfree airが出現し，軸捻は解除されていた．胃軸捻に続発した胃穿孔の診断で，当院へ転院搬送された．腹部は板状硬で，上腹部に圧痛を認めた．緊急手術所見では，腹腔内に混濁した血性腹水を認め，胃体中部後壁に径10mmの穿孔を認めた．胃穹窿部の固定は不良で胃脾間膜が伸長し，胃穹窿部が尾側へ移動することで容易に胃前庭部が穹窿部の頭側へ軸捻した．軸捻による屈曲部に一致した穿孔部位を縫合閉鎖し，再発予防目的に胃穹窿部を左横隔膜に縫合固定した．術後経過は良好で，第10病日に軽快退院した．胃穿孔をきたした成人の特発性胃軸捻は稀で，若干の文献的考察を加えて報告する．

症例は78歳，女性．臀部皮膚腫瘍切除術を施行，血管腫と診断された．同時期のCTで右肺下葉に結節性病変を指摘され，増大したため肺部分切除術を施行．病理診断は血管腫であったが，臀部病変との関連は明らかでなかった．初回手術から1年後のCTで肝右葉腫瘤を認め，経時的な増大を認めたことから右葉切除を計画し，残肝容量不足のため，開腹門脈右枝結紮と腫瘍生検を施行した．

A Case of Accessory Seminal Vesicle in the Rectal Mesentery

症例は69歳の男性．CTで直腸間膜内に，12mm大の境界明瞭で造影効果を伴う結節性病変を認めた．MRIではT1強調画像で低信号，T2強調画像で高信号，拡散強調画像で高信号を呈し，PET-CTではSUV-max 2.2の軽度集積を認めた．EUS-FNAを施行した結果，核腫大，クロマチン増量した異型細胞が散在しており，癌が疑われる組織像であった．免疫組織染色では，CK7（+），CK20（-），CD20（-），desmin（-），S-100（-），c-kit（-），synaptophysin（-），chromogranin A（-）であり腺癌が疑われ，原発性の直腸間膜内腫瘍が考えられたが質的診断には至らず，腹腔鏡下腫瘍摘出術を施行した．腫瘤は直腸間膜内の孤立した病変であった．12mm大の黄白色で境界明瞭な充実性腫瘤であり，病理では，リポフスチン顆粒をもち不鮮明な二層性をもつ腺管とestrogen receptor陽性の平滑筋成分からなる病変であった．男性の骨盤内の腫瘤であり，副精嚢と診断した．直腸間膜内に認めた副精嚢に対する1切除例を経験した．孤立した副精嚢は極めて稀であり，若干の文献的考察を加えて報告する．

Progressive Severe Clostridium difficile Infection after Abdominoperineal Resection

症例は63歳の女性で，特記すべき既往歴はなかった．直腸癌に対し，腹会陰式直腸切断術を施行し術後第10病日に退院した．術後第12病日から水様便の増加を認めたため術後第14病日に緊急入院となった．血液検査ではWBC 37,200/μl，CTでは横行結腸から下行結腸にかけて浮腫を伴う拡張を認め，便培養でClostridium difficile感染症（Clostridium difficile infection；以下，CDIと略記）と診断した．抗生剤治療を行ったが同日，CT所見の増悪，全身状態の著しい悪化を認め，入院24時間後に重症CDIによるショックと診断し緊急で，大腸全摘，回腸人工肛門造設術を施行した．摘出検体では横行結腸～下行結腸に偽膜付着を伴う小潰瘍が多発していた．術後集中治療を行ったが，術後30時間後に多臓器不全で永眠された．術前CDI発症リスクの低い患者においても重症CDIにより致死的な経過を辿ることがあり，術後下痢症状に留意すること，年齢や白血球数の推移など考慮した手術治療の早期決断・介入が重要であると考えた．
