Variability in Cerebral Autoregulation in Children with Acute Heart Failure Transitioned from Venoarterial Extracorporeal Membrane Oxygenation (VA-ECMO) to Ventricular Assist Device (VAD)

Aidan Crozier, Celia Gagnon, Elizabeth M. Malick, Alexis Schulz et autres

We report on an exploratory assessment of cerebral hemodynamics and autoregulatory impairment using non-invasive diffuse optical monitoring during extracorporeal membrane oxygenation and transition to the Berlin Heart EXCOR ventricular assist device in 3 pediatric patients.

Quantification of Cerebral Autoregulation in Neonates Undergoing the Norwood Procedure

Nicolina R. Ranieri, Rodrigo Forti, Tiffany S. Ko, Alyssa Seeney et autres

Diffuse correlation spectroscopy can quantify cerebral blood flow and assess autoregulation during neonatal cardiac surgery. After cardiopulmonary bypass, impaired autoregulation was more likely among neonates who underwent deep hypothermic circulatory arrest instead of selective cerebral perfusion.

Abstract 4367514: Association of Cardiopulmonary Resuscitation Strategies with Diffuse Optical Measurements of Cerebral Hemodynamics

Darci E. Anderson, Jeremy L. Herrmann, Kumaran Senthil, Aidan Crozier et autres

Introduction: Hemodynamic-directed cardiopulmonary resuscitation (HD-CPR) targets blood pressure (BP) goals during resuscitation and has shown improved outcomes compared to conventional algorithmic CPR. However, the relationship between specific BP targets and cerebral health remains poorly understood. Objective: Evaluate the association of intra- and …

Abstract Sat206: Differences in Cerebral and Somatic Hemodynamics during Asphyxia and Cardiac Arrest and Associations with Return of Spontaneous Circulation

Tiffany S. Ko, Darci E. Anderson, Kumaran Senthil, Jeremy L. Herrmann et autres

Introduction: We have developed non-invasive optical devices for concurrent monitoring of cerebral and somatic hemodynamics using frequency-domain diffuse optical spectroscopy (FD-DOS) and diffuse correlation spectroscopy (DCS). Objectives: In a pediatric swine model of asphyxia-associated cardiac arrest, compare cerebral and somatic hemodynamics during …

154: Changes in Cerebral and Peripheral Oxygenation and Blood Flow Following VA to VAV ECMO Conversion in the Setting of Pediatric Differential Hypoxemia

Elizabeth M. Malick, Aidan Crozier, Alyssa Seeney, Darci E. Anderson et autres

Background: Children requiring ECMO support after Fontan completion represent a high-risk population with previous ELSO data (1987-2009) demonstrating low survival rates of 35%. Methods and Objectives: Retrospective analysis of children undergoing their first ECMO run after Fontan completion between 2010 and 2023. …

320: Intraoperative Optical Monitoring of Cerebral and Somatic Oxygenation and Perfusion during Transition from VA-ECMO to Berlin Heart VAD

Aidan Crozier, Elizabeth M. Malick, Alyssa Seeney, Darci E. Anderson et autres

Background: In 2010, a system to rapidly deploy the (Extracorporeal Membrane Oxygenation) ECMO team was implemented utilizing an in-house emergency response platform (VECOM). In 2020, our institution launched a quality improvement (QI) initiative to standardize nomenclature and refine workflows resulting in enhanced …

Association of Arterial Blood Pressure Goals during Cardiopulmonary Resuscitation with Diffuse Optical Measurements of Cerebral Hemodynamics

Darci E. Anderson, Aidan Crozier, Kumaran Senthil, Jeremy L. Herrmann et autres

We utilize non-invasive diffuse optical neuromonitoring to quantify cerebral hemodynamics during hemodynamic-directed cardiopulmonary resuscitation (HD-CPR) strategies that maintained either higher or lower arterial blood pressure targets in a pediatric swine model of asphyxia-associated cardiac arrest.

Abstract Su1005: Differences in optical metrics of cerebral autoregulation after pediatric cardiac arrest

April M. Hurlock, Alyssa Seeney, Nicolina R. Ranieri, Ryan Burnett et autres

Introduction: Cerebral autoregulation (CAR) mechanisms, which maintain cerebral blood flow (CBF) over a range of mean arterial pressures (MAP), are impaired after cardiac arrest. Metrics of CAR impairment can be calculated from MAP and a CBF surrogate. Brain tissue oxygenation (StO 2 …

Abstract Sa804: Brain-Directed Management of Venoarterial Extracorporeal Membrane Oxygenation in a Pediatric Swine Model

Tiffany S. Ko, Katsunari Terakawa, Benjamin F. Smood, Julia Slovis et autres

Introduction: We present a novel demonstration of brain-directed management of venoarterial extracorporeal membrane oxygenation (ECMO) using real-time feedback from non-invasive neurometabolic optical monitoring (NOM) of the cerebral oxygen extraction fraction (OEF) in pediatric swine. In our prior work, OEF>0.82 was associated with …

Novel Optical Neuromonitoring of Cerebral Oxygen Delivery During Antegrade Cerebral Perfusion in Neonates

Nicolina R. Ranieri, Rodrigo M. Menezes Forti, Tiffany S. Ko, Alyssa Seeney et autres

We demonstrate the feasibility of novel diffuse optical neuromonitoring to continuously quantify cerebral blood flow and oxygenation during antegrade cerebral perfusion in neonates with hypoplastic left heart syndrome during the Norwood procedure.
