Aortopulmonary Fistula after 15 years after a Modified Bentall Procedure

症例は72歳男性で，15年前に大動脈弁輪拡張症 (AAE) に対してCarrel patch法による大動脈基部置換術を施行されたあと，左右の冠動脈ボタン吻合の破綻が原因の仮性動脈瘤と大動脈肺動脈瘻を発症した．再手術を行い，瘻孔は主肺動脈に認められた．肺動脈の欠損孔はテフロンフェルトを通した 3-0 ポリプロピレン糸の結節縫合で閉鎖した．左右の冠動脈ボタンは人工血管より完全に遊離していた．Composite graftの冠動脈ボタン孔は人工パッチにより閉鎖した．大伏在静脈グラフト (SVG) による左前下行枝 (LAD) および右冠動脈 (RCA) への冠動脈バイパス術 (CABG) を施行した．Modified Bentall術後の冠動脈吻合部仮性瘤による肺動脈への穿破はまれな合併症であり，数例報告があるのみであった．
