Provision of intensive care for an adult with coronavirus disease 2019 at a freestanding children’s hospital

新型コロナウイルス感染症（coronavirus disease 2019, COVID-19）患者の急増により成人ICUの病床が逼迫し，独立病院型小児総合医療施設である当院に成人COVID-19患者の受け入れ要請があった。患者は75歳，男性，発熱後にCOVID-19と診断され呼吸困難のため前医に入院した。その後呼吸不全に至り，気管挿管後に当院PICUに転院した。2日間の筋弛緩薬持続投与と腹臥位療法を併用した肺保護換気を行い，酸素化・静的呼吸器系コンプライアンスは改善したが，大きな1回換気量と人工呼吸器関連肺炎が懸念され，他院の成人ICU医師に相談しながら抜管の可否を日々評価した。転院5日後に抜管，転院7日後に前医に転院した。 本症例の受け入れには，事前の準備，病院内外との連携やスタッフの教育による成人診療の質の確保，小児医療体制の維持などの様々な課題に取り組む必要があったが，小児重症患者に対する医療体制を維持しながら十分な質の成人医療を提供し得た。
