A Case of Erosive Pustular Dermatosis of the Scalp Due to Osimertinib

68 歳，女性。右上肺腺癌に対して，オシメルチニブ，ベバシズマブ投与開始 9 日目に顔面に Grade 2 のざ瘡様皮膚炎が出現し，爪周囲炎，下痢，皮膚乾燥が続発した。投与開始 17 カ月から後頭部に鱗屑，膿疱，びらん，脱毛を伴う紅斑局面が出現し，徐々に症状が増悪したため当科を紹介され受診した。皮膚生検の病理組織像では，真皮浅層から深層にかけて，主に毛包周囲に好中球，リンパ球，形質細胞の稠密な炎症細胞浸潤を認め，真菌要素は陰性であった。以上より Erosive Pustular Dermatosis of the Scalp（EPDS）と診断した。ベタメタゾン酪酸エステルプロピオン酸エステル液外用を開始したところ，皮疹ならびに脱毛は徐々に改善し，治療開始 6 カ月後には十分な発毛が得られたため外用治療は終了した。6 カ月後，頭皮に脱毛を伴わない紅斑，丘疹，痂皮が生じ，症状の再燃と判断し外用治療を再開した。近年，epidermal growth factor receptor（EGFR）阻害薬投与による EPDS 発症報告が散見されており，その発症機序や臨床的特徴について検討した。
