A Case of Pemphigus Vegetans

61 歳，男性。初診 3 週間前より口唇と口腔内にびらんが出現した。2 週間前より股部と陰部の間擦部に暗紅色の増殖性局面が出現したため受診した。病理組織学的所見で表皮内に棘融解を認め，蛍光抗体間接法で抗表皮細胞膜抗体が陽性，ELISA 法で抗デスモグレイン 3 抗体が陽性であり，臨床所見と併せて増殖性天疱瘡と診断した。プレドニゾロン（PSL）60 mg/日で治療を開始し，皮疹は速やかに改善した。増殖性天疱瘡は，尋常性天疱瘡の亜型で，近年では抗デスモコリン抗体の関与が指摘されている。自験例では抗デスモコリン 1/2/3 抗体は全て陰性だったが，抗デスモコリン抗体陽性の増殖性天疱瘡の報告もあり，病態への関与についても考察した。
