A Case of 9-Year Disease-Free Survival after Pancreaticoduodenectomy for Local Recurrence in the Pancreatic Head after Surgery for Early-Stage Gastric Cancer

症例は58歳の男性で，2012年7月胃癌に対して開腹幽門側胃切除術を施行した．病理所見はL，Less，tub2>por1，pT1b（SM2）N0M0，pStage IAであり，術後は経過観察を行った．術後12か月のフォローCTにて膵頭部腫瘤性病変を認め局所再発と診断した．腫瘍マーカーの上昇は認めず，画像上その他の再発所見は認めなかったため，2013年8月開腹膵頭十二指腸切除＋肝部分切除＋横行結腸部分切除を施行した．再手術後，術後補助化学療法として1年間S-1の内服を行い，その後は経過観察を継続し現在再発切除後9年間，無再発生存中である．局所に限局しており，根治的切除が可能であれば，胃癌局所再発に対しても手術も治療の選択肢になると考えられた．

A Case of Adenocarcinoma with Enteroblastic Differentiation at the Esophagogastric Junction

症例は61歳の男性で，食後のつかえ感を主訴に近医を受診し，当院へ紹介となった．食道胃接合部に1型腫瘍を認め，生検で中分化型腺癌と診断し，胃全摘術，腹部食道切除術を施行した．病理組織学的検査所見では，異型円柱上皮が乳頭状構造をとる高分化型腺癌を認め，淡明な細胞質を持ち，核異型は高度であり，免疫染色検査でAFP陽性，Glypican3陽性を示し，胎児消化管類似癌と診断した．リンパ節転移はみられなかったため，術後補助化学療法は施行せず，経過観察としていた．術後2年目の胸腹部CTで右肺転移を認めたため，右肺下葉切除術を施行した．病理組織学的検査でも，食道胃接合部癌の転移として矛盾しない所見であった．本人の希望で術後補助化学療法は行わず経過をみていたが，肺転移切除後1年10か月無再発生存を得られている．今回，我々は食道胃接合部に生じた大変まれな組織型である胎児消化管類似癌を経験したため報告する．
