58 歳，男性．気管支喘息で加療中の方．2 か月前から咳嗽出現し，胸部単純 X 線写真で両側に浸潤影を認めた．気管支鏡検査を施行し気管支肺胞洗浄液で好酸球数の増加を認め，また内腔所見では気道上皮に白苔を認め，病理所見でびらん，潰瘍を伴う好酸球浸潤を認めた．慢性好酸球性肺炎と診断し，ステロイド投与を行い改善を認めた．気管支潰瘍を伴った好酸球性肺炎は稀であり若干の文献的考察を加えて報告する．
