症例は71歳，男性．左下腹部のしこりを契機にS状結腸癌と診断され，精査目的のCT検査にて胃小弯側に47mm大の腫瘤を指摘された．胃GISTを疑い腹腔鏡下S状結腸切除および腫瘤摘出術を施行した．腫瘍は左胃動脈と胃小弯の間に存在し，胃壁との連続性はなかった．病理組織検査では不規則に配列・増殖した紡錘形細胞からなり，免疫組織染色ではc-kit陽性，CD34陽性，S-100 protein陰性，αSMAは陰性で，核分裂像数は5個以下/50HPF，Ki-67陽性率は2-3％であった．以上より小網原発GISTと診断し，modified Fletcher分類における低リスク群であった．術後経過は良好でS状結腸癌に対する術後補助化学療法を施行し経過観察中である．消化管外に発生したGISTは稀な疾患であるが，手術の際にはGIST再発を念頭に置いた術式選択が必要である．
