Tc-99m-methoxy-isobutyl-isonitrile（MIBI）シンチグラフィで取り込みのない巨大な機能性副甲状腺囊胞の1例を報告する。症例：74歳女性。口渇や全身倦怠感を自覚し，高Ca血症，int-PTHの高値を指摘され受診した。単純CT検査，頸部エコー検査で，右頸部に囊胞性の腫瘤を，左頸部甲状腺内とその背側に充実性の腫瘤を認めた。右頸部囊胞内容液中のint-PTHは高値であったが，MIBIシンチでは左頸部に異常集積を認め，同部位の摘出手術を行った。永久病理標本にて右機能性副甲状腺囊胞，左腺腫様甲状腺腫，左副甲状腺過形成と診断した。術後1年を経過したものの経過は良好である。

A Case of Littoral Cell Angioma Incidentally Found with Non-traumatic Splenic Rupture

非外傷性脾破裂に対して脾臓摘出術を施行し，術後病理診断でlittoral cell angioma（以下，LCA）と診断された1例を経験したので，若干の文献的考察を加えて報告する．症例は50歳，男性．発熱，左上腹部痛，左肩への放散痛を主訴に当院を受診した．来院時の血液生化学検査では，炎症反応の上昇を認めたが，明らかな貧血所見などは認めなかった．腹部超音波検査にて脾臓内部に不均一な像を認めた．腹部造影CT検査では内部に淡い高吸収域を伴う腫大した脾臓を認め，脾臓周囲から左横隔膜下にかけて血腫がみられた．骨盤内には腹水の貯留を認めた．非外傷性脾破裂と診断し，脾臓摘出術を施行した．病理組織学的検査ではLCAと診断された．

A Case of Appendiceal Endometriosis Associated with a Mucocele

症例は38歳，女性．繰り返す心窩部痛と右下腹部痛を主訴に前医を受診し，腹部超音波検査にて虫垂腫大を指摘され当院受診となった．腹部CTでは虫垂の嚢胞性腫大を認め，下部消化管内視鏡検査や注腸造影検査では，盲腸を圧排する粘膜下腫瘤を認めた．虫垂粘液腫の術前診断で，腹腔鏡下回盲部切除+D2郭清を施行した．病理組織学的検査にて虫垂子宮内膜症に起因する粘液腫の診断となった．閉経前女性の虫垂粘液腫では，虫垂子宮内膜症が原因となる可能性を念頭に置くことが重要と思われる．
