［目的］透析妊婦は切迫流産に対する塩酸リトドリンの使用頻度が高いが透析除去率の報告はない．塩酸リトドリンを投与した透析妊婦で塩酸リトドリンクリアランス（CL），除去率，ダイアライザCLを計算した．［症例］36歳女性，透析歴5年．妊娠20週4日から切迫兆候を認め塩酸リトドリン投与．血液透析（HD）直後に症状悪化し，透析時に約25％増量投与し血液濾過透析（HDF）に変更，27週6日で経腟分娩に至った．［方法］HD（APS-15MD，QB 180 mL/min，QD 500 mL/min），HDF（サブラッドBD8L® （前希釈），APS-15MD，QB 180 mL/min，QD 450～500 mL/min，QF 33 mL/min）のダイアライザ前後，HDFで透析前，2時間目，終了4時間目に採血施行．塩酸リトドリンを非透析時126 μg/min，透析時160 μg/minで投与し患者側血中濃度，ダイアライザ出口側濃度，ヘマトクリット，血液流量および濾液流量を用いて，血漿基準CLを算出した．［結果］HDF時の塩酸リトドリン血中濃度は透析前62.03 ng/mL，2時間目69.08 ng/mL，終了後68.48 ng/mLでありダイアライザ前後ではHD時前68.52 ng/mL，後40.35 ng/mL，血漿CL 52.3 mL/minであった．HDF時はダイアライザ前75.82 ng/mL，後で37.10 ng/mLであり血漿CL 81.3 mL/minであった．［考察］HD，HDFいずれも塩酸リトドリン透析性は過去の報告から想定される透析性と比較して低かった．しかし塩酸リトドリン増量投与により妊娠期間が継続したことから，薬剤の透析性とは別に透析療法そのものが子宮収縮抑制剤の需要量を増すと考えられ，透析妊婦に塩酸リトドリンを投与する際は透析による切迫兆候悪化予防に増量投与する必要があると考えられた．
