Acute Focal Bacterial Nephritis Caused by a Finger Penile Clamp for Voiding Postponemennt

症例は9歳男児．発症当日に倦怠感，嘔吐が出現し，翌日に当科を受診した．ウイルス性胃腸炎として点滴補液したところ，症状は改善したが，その後も発熱を繰り返していた．発症12日目，右肋骨脊椎打痛陽性であり，造影computed tomographyを撮影し右急性巣状細菌性腎炎の診断に至った．尿培養からはEnterococcus faecalisが検出された．排尿時膀胱尿道造影で右にGrade Ⅲの膀胱尿管逆流を認めた．詳細な問診から就学後頃から家庭で用手的penile clampによる習慣性排尿延期の実態が明らかになった．幼児期にトイレットトレーニングが無事終了していても，学童期以降に不適切な排尿習慣が形成され，重症な尿路感染症の誘因となりうるため，啓発活動に努める必要がある．
