We report the case of a 77-year-old patient with glioblastoma, IDH-wildtype, presenting as adult primary leptomeningeal gliomatosis with granular cell morphology in the biopsy. The clinico-radiological presentation initially suggested a non-neoplastic granulomatous process. Comprehensive molecular studies and post-mortem examination were subsequently performed.
