A Case of Toxic Shock Syndrome Caused by Burns on Both Lower Legs

42 歳，女性。天ぷら油で両側下腿に 7％のⅠ～Ⅱ度熱傷を受傷した。当初，外来にて通院治療を開始したが，自宅処置が困難と訴えがあり，処置目的に受傷 3 日目に入院した。全身状態は問題なかった。受傷 4 日目に，突然 39 度の発熱，嘔吐，下痢が出現し，わずか数時間で，意識障害，急速な血圧低下，全身の潮紅を認め，多臓器不全となり，集中治療室での管理を始めた。臨床症状と検査所見より toxic shock syndrome（TSS）と診断した。抗菌薬，大量免疫グロブリン投与が奏効し，全身状態は改善した。本症例は，創部培養より黄色ブドウ球菌が検出されたものの，創部に感染徴候はなく，軽症熱傷を契機として発症した TSS だった。熱傷治療において，予防的抗生剤投与は推奨されていない一方，TSS には有効性を示した報告もあり，受傷早期の抗生剤投与については一定の見解が得られていない。TSS は急激に状態が悪化するため，診断や治療が遅れた場合，死亡する場合もあり，数時間以内の迅速な診断が求められる。且つ患者年齢や基礎疾患，熱傷の重症度によらず発症する可能性があることを留意すべきである。
