大部分の両側多嚢胞性異形成腎 (MCDK) はPotter症候群を呈し,周産期に致死的経過をとる。周産期死亡を免れ,現在腹膜透析にて管理しているまれな両側MCDKの1例を報告する。 母親は糖尿病のためインスリン治療をしていたが,血糖のコントロールが不良であった。在胎18週に胎児エコーで両腎に多数の嚢胞を認めたが,羊水量は正常であった。在胎33週に羊水過少を来し,帝王切開で出生した。腹部エコー,MRIで両側MCDKと診断した。左腎の一部に腎実質を認め,DMSAシンチグラフィーで左腎下極に取り込みを認めたため,左腎はsegmental MCDKと診断した。腎機能が徐々に悪化し,生後4ヵ月から腹膜透析を導入した。1歳2ヵ月で成長・発達は正常範囲内にある。本例では左腎下極に残腎機能を有していたために周産期死亡を免れえた。母親に糖尿病があり胎内での高血糖あるいはTCF2遺伝子等の異常が発症に関与した可能性があり,遺伝子検索を含めたさらなる検討を要すると思われた。
