全身性エリテマトーデス（systemic lupus erythematosus: SLE）の合併症のひとつにループス腹膜炎がある．SLEの活動期に一致して腹膜刺激症状などを呈する病態だが，小児例での報告はわずかである．症例は14歳女子．腹部膨満，下腿浮腫，腹痛を主訴に受診した．血液検査で低アルブミン血症，低ナトリウム血症，血小板減少を認め，精査加療目的に入院した．低補体血症を認めたが，dsDNA抗体や尿蛋白は陰性であった．画像検査では著明な腹水貯留を認めた．ステロイド治療により経時的に腹水は減少し，検査値の改善も得られた．その後腎生検にてループス腎炎（International Society of Nephrology/Renal Pathology Society ［ISN/RPS］）class III（A）を認め，SLEと診断した．ステロイドパルス療法，ヒドロキシクロロキン，ミコフェノール酸モフェチルによる治療を行い，寛解が得られた．本症例はSLE診断基準に合致しない部分もあり，診断に難渋した．早期診断のためには，ループス腹膜炎に対する理解と腎生検による評価が重要である．
