55歳，男性．難聴の精査目的に施行した頭部MRIで偶発的に外包を含む左右対称性の白質病変を指摘された．臨床症状は軽微であったが，病変の分布と家族歴からCADASILが疑われた．NOTCH3遺伝子に既知の変異（p.R75P）を認め診断が確定した．変異のタイプによっては，一定の年齢に達しても古典的なCADASILの臨床像が認められないため，臨床症状に乏しくとも外包を含む特徴的な病変を認めた場合にCADASILを念頭に置く必要がある．
