Two Cases of Ulcerative Colitis with Left Renal Vein Thrombosis Preoperatively

潰瘍性大腸炎(ulcerative colitis;以下,UCと略記)の腸管外合併症である静脈血栓塞栓症では深部静脈血栓症が多いが,左腎静脈に血栓形成することはまれであり,合併すると予後不良である.当科では致死性合併症併存時のUC手術治療として3期分割手術を基本とし,初回手術として大腸亜全摘術,回腸人工肛門造設を行う方針としている.今回,術前に左腎静脈血栓を合併したUC症例2例を経験したので報告する.症例1は26歳の男性で,内科治療抵抗性のUC術前に左腎静脈血栓を合併した.症例2は57歳の男性で,寛解期のUCを背景としたS状結腸癌術前に左腎静脈血栓を合併した.腎静脈血栓の場合,下大静脈フィルターは腎血流保護のため留置困難で,2症例とも腹腔鏡下大腸亜全摘術を施行後,速やかに抗凝固療法を行うことで,術後静脈血栓塞栓症関連合併症を併発することなく治療しえた.
