要旨：【目的】ステントリトリーバー導入後の当院での急性期血行再建術の治療成績を後方視的に 検討し，特に非再開通症例に焦点を当て，その原因を考察した．【方法】急性期脳梗塞に対し血行再 建術を施行した 55 症例を対象に治療成績を報告し，再開通群と非再開通群とに分け比較検討した． 【結果】全 55 例中 48 例（87％）に有効な再開通が得られ，非再開通例は 7 例（ICA 閉塞 5 例，MCA 閉 塞 2 例）であった．ICA 閉塞例では併用デバイスを要することが多く治療が複雑化，長時間化してい た．MCA 閉塞例では高度屈曲部でのステントリトリーバーの展開不良が確認された．【結語】治療成 績は向上しているが，非再開通例も存在する．ICA 閉塞例では治療の複雑化，長時間化が，MCA 閉 塞例では屈曲部でのステントリトリーバー展開不良が再開通困難の原因と思われた．
