症例は66歳男性．大腸癌に対する5-FUをベースとした全身化学療法中（FOLFILI＋パニツムマブ）に食欲不振，倦怠感と急激な腹水の貯留を認めた．血液生化学所見では肝逸脱酵素の上昇と低アルブミン血症，PTの低下を認めた．画像診断では著明な脂肪肝を認め薬物性の肝障害，脂肪肝と考えられた．薬物の中断，利尿剤の投与等を行ったところすみやかに症状，肝機能の改善，腹水の消失，画像上の脂肪肝の消失を認めた．発症1年前の肝切除標本においては背景肝に脂肪化，肝細胞障害，線維化，炎症所見等は認めなかった．発症2カ月前のCTにおいても肝臓に異常所見を認めなかった．3回の肝切除後の肝容積の回復は良好で5年9カ月の全経過の後，間質性肺炎で死亡するまで肝転移の再発は認めなかった．臨床経過からイリノテカンを被疑薬とする薬物性急性脂肪肝と考えられた．
