Plasma exchange therapy inhibited the progression of anti-MAG antibody-associated polyneuropathy

症例は75歳男性で, 四肢遠位部のしびれを認めた. myelin-associated glycoprotein抗体陽性ニューロパチー (MAGN) と診断され, 経静脈的免疫グロブリン療法 (IVIg) やリツキシマブ, ステロイドパルス療法, アザチオプリンを導入するも改善は認められなかった. 血漿交換療法 (PE) を導入したところ, 症状の改善を認めた. 2か月おきに約4年間でPEを計26コース施行し, 症状の進行を認めなかった. 近年MAGNに対する治療として, リツキシマブに関するrandomized controlled trialが散見され, 治療の中心となりつつあるが, 3分の1で無効ないし悪化するとの報告もある. PEは短期間での有効性を認めた報告例はあるが, 本症例のように長期にわたり状態の維持を認めた報告はなく, PEが維持療法として有用である可能性が考えられた.
