Pleuroperitoneal communication following initial peritoneal dialysis after renal graft failure： A case report

症例は42歳女性．先天性腎尿路異常（CAKUT）から末期腎不全に至り，X‒19年に生体腎移植を施行した．その後ステロイドを含む免疫抑制剤を内服していたが，腎機能が悪化し，X年に持続携行式腹膜透析（CAPD）を開始した．初回腹膜透析液注入後から，胸部違和感が出現し，胸部X線で右片側胸水を認めた．CT peritoneographyで，横隔膜欠損孔を示唆する所見を認め，横隔膜交通症と診断した．第9病日に横隔膜縫縮術を施行し，第15病日にCAPDを再開した．翌日に胸部違和感が出現し，胸部X線で右胸水の増加を認めた．横隔膜交通症の再発と考え，血液透析導入の方針とした．本例では，原疾患がCAKUTであったことから先天的に横隔膜欠損孔が存在していた可能性や，ステロイドの長期使用が横隔膜の脆弱性をきたしたことが，横隔膜交通症の発症，再発に寄与した可能性がある．移植腎機能廃絶時に腹膜透析を導入した際には，横隔膜交通症の発症に注意する必要があると考える．
