Abstract Introduction Precocious puberty is defined as the appearance of secondary sexual characteristics before age 8 in girls and 9 in boys. The LHRH stimulation test plays a crucial role in identifying the origin of precocious puberty and differentiating true cases from benign variants. Materials and Methods This cross-sectional study included 16 cases investigated for precocious puberty. Clinical and hormonal evaluations were performed for all patients. Results Twelve girls and four boys made up the 16 participants in the study. At consultation, the average age was five years old (range: one to nine years). Premature pubarche (n = 7), premature thelarche (n = 7), and increased testicular volume (n = 2) were the reasons for consultation. Every patient underwent the LHRH test. With LH peaks ranging from 5 to 94 IU/L and FSH/LH ratios between 0.63 and 4.9, the findings indicated a central origin in six patients. Five patients were diagnosed with pseudo-precocious puberty. Following the LHRH test, the LH peak varied from 0.5 to 3.9 IU/L, and the FSH/LH ratio was between 0.18 and 1. In 5 girls with a mean age of 3 years (1-5 years), premature thelarche was identified. The LHRH test showed LH peaks between 0.8 and 7.2 IU/L and FSH/LH ratios between 0.2 and 2.9. Conclusions Precocious puberty may indicate a serious pathology of central or peripheral origin. However, isolated premature thelarche can be considered a normal variant, highlighting the importance of the LHRH test in distinguishing true precocious puberty from simple premature puberty.
