Results: The proposed Particle Therapy Center comprised two PBT and one BNCT treatment rooms.Patient demand forecasting estimated annual utilization of approximately 600-800 patients for PBT and 250 patients for BNCT to sufficiently support phased implementation and stable service uptake.Estimated capital investment costs were ~$140 million.PBT and BNCT demonstrated financial viability under base-to-high utilization scenarios.Under base-case conditions, PBT achieved a PP of ~13 years, with a positive NPV and IRR of 11.4%.BNCT demonstrated a PP of 18.5 years, with a positive NPV and IRR of 6.8%.Risk assessment identified 28 risks across strategic, operational, financial, and compliance domains, with 26 classified as moderate-to-high severity.Key risk drivers included capital investment burden, workforce readiness, regulatory requirements, and patient demand variability.Conclusions: Establishment of a comprehensive Particle Therapy Center integrating PBT and BNCT is operationally and financially feasible; however, systematic and proactive risk management is essential to ensure long-term sustainability.This initiative could enhance equitable access to advanced particle therapy and strengthen national oncology capacity for academically driven particle therapy services, offering a scalable model for other middle-income health systems in SEA region.
