Early diagnosis of multiple sclerosis in the context of the 2024 McDonald criteria: from clinically and radiologically isolated syndromes to biological verification of the disease

This article examines the evolution of diagnostic criteria for multiple sclerosis (MS), with a focus on the 2024 revision of the McDonald criteria as a milestone in the diagnosis of the disease. A comprehensive analysis of changes in MS diagnostic approaches is presented, demonstrating a shift from reliance on demonstration of dissemination in time (DIT) to confirmation of dissemination in space (DIS) combined with highly specific biological markers. The study describes the search methodology in detail. The final selection included 239 studies after screening and removal of duplicates and irrelevant articles. Practical aspects of implementing the new criteria in clinical practice are discussed, including early-stage diagnosis (clinically and radiologically isolated syndromes), differential diagnosis in special patient groups (children, patients over 50 years of age, and those with cerebrovascular pathology), prospects for early therapy, and medication selection. The 2024 McDonald criteria are shown to improve the accuracy and speed of MS diagnosis, reduce the risk of overdiagnosis, and enable earlier treatment initiation. The need for interdisciplinary collaboration among specialists (neurologists, radiologists, and laboratory diagnosticians) and standardization of research methods to realize the full potential of the updated criteria is emphasized.
