Bayesian reanalysis of the PROfound trial: Olaparib in Asian patients with metastatic castration-resistant prostate cancer

Objective The PROfound trial investigated the efficacy of the poly(adenosine diphosphate–ribose) polymerase inhibitor olaparib in metastatic castration-resistant prostate cancer with homologous recombination repair gene alterations, particularly in Cohort A ( BRCA1/2 or ATM alterations). However, the Asian subgroup analysis of Cohort A was statistically non-significant with wide confidence intervals. This study aimed to explore another perspective by applying a Bayesian approach to post hoc analysis. Methods We performed a Bayesian reanalysis of the Asian Cohort A ( n =59) using published data, borrowing information from the global Cohort A ( n =245) under four prior-specification scenarios: neutral (no borrowing), conservative, moderate, and strong borrowing. Posterior hazard ratios (HRs), 95% credible intervals (CrIs), and posterior probabilities of benefit ( p [HR<1], p [HR<0.8]) were estimated for radiographic progression-free survival and overall survival. Results For radiographic progression-free survival, posterior HRs ranged from 0.618 (95% CrI 0.316–1.210) with the neutral prior to 0.371 (95% CrI 0.278–0.495) with strong borrowing. Even without borrowing, p (HR<1) was 92.0% and p (HR<0.8) was 77.4%. With conservative borrowing, p (HR<1) was >99.9%, whereas p (HR<0.8) was 99.5%; with moderate and strong borrowing, p (HR<1) and p (HR<0.8) were each >99.9%. For overall survival, posterior HRs ranged from 0.831 (95% CrI, 0.418–1.650; neutral) to 0.709 (95% CrI, 0.524–0.959; strong). p (HR<1) increased from 70.1% (neutral) to 88.3% (conservative), 93.7% (moderate), and 98.2% (strong), whereas p (HR<0.8) ranged from 45.6% (neutral) to 78.4% (strong). Conclusion Although constrained by statistical limitations, including prior dependence and the dominance of the larger global cohort, this analysis illustrates the utility of Bayesian methods in complementing frequentist approaches and offers a probability-based perspective supporting the efficacy of olaparib in Asian patients with homologous recombination repair – mutated metastatic castration-resistant prostate cancer.
