Hemorrhagic acute cholecystitis is a rare and severe form of acute cholecystitis, characterized by intraluminal gallbladder hemorrhage, mural ischemia, and an increased risk of local and systemic complications.Its association with Mirizzi syndrome and biliary-enteric fistula may lead to upper gastrointestinal bleeding, representing an atypical and diagnostically challenging clinical presentation.We report the case of a 61-year-old male patient with significant cardiovascular comorbidities and chronic oral anticoagulation, diagnosed with hemorrhagic lithiasic acute cholecystitis complicated by a cholecystoduodenal fistula and Mirizzi syndrome.A combined surgical and staged endoscopic approach resulted in a favorable clinical outcome.
