Abstract This chapter presents a 44-year-old man with seizures and altered mental status, whose lumbar puncture reveals a dry tap followed by bloody, xanthochromic cerebrospinal fluid with markedly elevated protein, consistent with the Froin-Nonne sign. His medical history includes childhood medulloblastoma, radiation-induced meningiomas, seizures, intellectual disability, and immunocompromise, all of which complicate the diagnostic picture. MRI findings suggest multiple meningiomas, pachymeningeal and leptomeningeal enhancement, and cortical abnormalities consistent with contiguous encephalitis. The Froin-Nonne sign reflects cerebrospinal fluid stagnation due to blockage, and the case warrants empiric treatment with broad-spectrum antibiotics and corticosteroids. Finally, the chapter emphasizes urgent diagnostic efforts in immunocompromised patients, including repeat lumbar or C2 puncture and biopsy to identify treatable infectious, neoplastic, or inflammatory causes.
