Abstract This chapter presents an 88-year-old man with diffuse large B-cell lymphoma, Parkinson disease, and renal failure who develops four limbs shaking and encephalopathy following antibiotic exposure. His cerebrospinal fluid shows marked pleocytosis, low glucose, and high protein, while blood cultures grow Listeria, and EEG reveals bilateral slowing with triphasic waves. Clinicians consider multiple causes for his altered mental status, including meningitis, renal dysfunction, and beta-lactam neurotoxicity, which likely results from GABA receptor antagonism. His symptoms match patterns seen in elderly patients with impaired clearance of beta-lactams, where symptoms typically improve after clinicians stop the drug, initiate antiepileptics, or begin dialysis. Finally, the chapter emphasizes early recognition of antibiotic-induced neurotoxicity, the importance of tailored antimicrobial and seizure management, and the need to balance aggressive treatment with comfort measures in medically fragile patients.
