Late Breaking Abstract - Bronchial Rheoplasty for Chronic Bronchitis: Results from the Randomized, Mechanical Sham-Controlled RheSolve Study

Introduction: Chronic bronchitis (CB), defined by persistent cough and mucus production, is a common chronic obstructive pulmonary disease (COPD) phenotype with a high symptom burden. Bronchial rheoplasty (BR) with RheOx® disrupts mucus and delivers pulsed electric fields (PEF) to airway epithelium to reduce mucus producing cells and CB symptoms. Methods: This randomized, double-blind study, conducted across 37 centers in the US and EU, randomized 273 patients 2:1 (BR:mechanical sham control), who were bilaterally treated bronchoscopically and followed for 12 months. The primary efficacy endpoint was the change in the COPD Assessment Test (CAT) from baseline to Month 6. Secondary outcomes included CB-specific patient reported outcomes (Cough and Sputum Assessment Questionnaire [CASA-Q]), distal airway volume (DAV), and goblet cell hyperplasia (GCH) on airway biopsy. Results: Bronchial Rheoplasty was not superior to mechanical sham for total CAT score at 6 months, with both groups demonstrating clinically significant improvements (BR -7.6 (±8.2) points vs. sham -7.0 (±7.8) points; P=0.643). In the per-protocol analysis BR was superior to mechanical sham-control with lower symptom scores specific to CB (CASA-Q sputum impacts, P=0.034), higher DAV (P=0.027), and lower GCH (P=0.023). The BR procedure was well-tolerated with a low rate of serious adverse events and no unanticipated AEs. Conclusions: BR and mechanical sham procedures were safe and resulted in substantial COPD symptom improvement, with no significant difference between groups due to the large response to the mechanical sham. BR was associated with improvements in CB-specific symptoms, airway histology, and DAV.
