EMBARC-BRIDGE: a representative biorepository to support translational research in bronchiectasis

The Bronchiectasis Research Involving Databases, Genomics and Endotyping (BRIDGE) study is a translational research subcohort of the EMBARC registry collecting extensive biosamples. We report characteristics of patients in the BRIDGE study compared to the EMBARC registry. Patients with bronchiectasis on HRCT ≥18yrs old from 19 European sites are included in BRIDGE at the time of writing. Data was entered into the EMBARC registry and samples taken for multi-omics and molecular characterisation. Between Aug 2019 and Feb 2025, 1102 patients were recruited to BRIDGE. Patient demographics were representative of the 16,963 individuals in the EMBARC registry: median age BRIDGE 66yrs (IQR 55–74) vs 67yrs (IQR 57–74), 61.3% female vs 60.9%. In BRIDGE and the registry, the most common cause of bronchiectasis was post-infective disease 18.4% and 21.2% respectively; 39.5% patients in BRIDGE were classified as idiopathic compared to 38.1% in the registry. Disease severity in BRIDGE was similar to that in the registry. Bronchiectasis severity index was median 6 points (4-8) vs 7 (4-10) and median FEV1 %pred 80.8% (63.0–98.4) compared to 76.9 (56.0–96.7). Patients in the BRIDGE study suffered less exacerbations per year at baseline: median 1 (IQR 0-2) vs 2 (IQR 2-4) in the registry. This is likely related to recruitment during the COVID-19 pandemic. Samples collected include sputum, nasal swabs, serum, DNA, urine and subcohorts collected nasal epithelial cells and other specialised cell types (e.g neutrophils). EMBARC-BRIDGE is the world’s largest biorepository of patient material collated to endotype bronchiectasis and is broadly representative of bronchiectasis across Europe as per the EMBARC registry.
