Background: Nitric oxide and reactive oxygen species are released in asthmatic airways, and the interaction between these substances induces oxidative and nitrative stress, which is involved in the intractability of asthma. While FeNO reflects nitrative stress in the airways, there is no method for evaluating oxidative stress, and in this study, we examined the involvement of Hcy in the pathology of asthma. Methods: A retrospective analysis was conducted on 40 patients, excluding 8 diabetic patients, out of 48 asthma patients who visited our hospital from April to August 2023. The correlation between Hcy and other factors was analyzed using Spearman's rank correlation coefficient. FeNO ≥ 22 ppb was used as the cutoff, and the patients were divided into two groups, a high group and a normal group, and Hcy values were compared using the Mann Whitney test. Factors affecting FeNO values (age, sex, BMI, Hcy value) were analyzed using multiple regression analysis. Results: Hcy was statistically significantly positively correlated with FeNO and V50/V25, and negatively correlated with %FEV1.0. On the other hand, Hcy had a weak positive correlation with age. The high FeNO group had significantly higher Hcy levels than the normal group. Hcy levels independently affected FeNO levels, even after adjusting for age, sex, and BMI. [Discussion] Hcy may be involved in the progression of asthma.
